Smelly or not - the aroma reaches a sniffing little nose and that belongs to a girl that occupies a seat not far away.
The girl's wrapped in a large coat, a bit too big, appearing yet bigger as it is made of something that looks like thick fur and as it is stuffed with candy, all sorts.
And candy is, what Lucy's had all day long. And now she smells cheese and it seems her eyes start eating ... be careful they don't eat away that sandwich (or at least the cheese).
Lucy reaches out with a smile that is a little embarrassed, yet big. After short hesitation she takes a little piece and chews with eyes closed and nose in the sky. Some "hmmms" and "aaahs" included. It takes a few moments until she swallows and opens eyes again.
"Wantcha candy?"
She offers a handful. Maybe hoping for more cheese.
there's some chewing in that. She pauses and offers a hand - pauses again, looking at it. Above being cheesy the hand's also covered with outdoor dirt, all stuck together with some chocolate. It's withdrawn again with a shrug.
"Oh I see. Things have changed from that in my world." he explains. "Back home we have what's called the National Health Service, which gives everyone free health care if they can't afford it."
Lucy nods and takes another piece of cheese. Enjoying every bit. It's not really hunger but pure pleasure. After swallowing, her attention returns to the doctor.
Lucy raises her head, smile's outbreaking again. It's easy to distract her.
"Oh - I've found this place several days ago ..."
In fact it could've been weeks, she's not good at time-management.
"It is soo gorgous. There's a lake and flowers and trees and ... even animals!"
Her hands dive into the deep pockets of her (still worn) Halloween-coat and come out with a handful of dry leaves, little cones and similar things. Just to prove. Then she stops, realizing that most people here seem to be very used to all that stuff.
Now that gets her climbing upon her seat, spreading some of her treasures on the table, careful enough not to get them into the rests of that wonderful cheese.

The 'doctor' gets a curious look. She's never met one before. She tells him that.

"You're not the first. Many people here haven't. Different worlds I suppose."
He sips his tea.

"In my world doctors never talk to ...people like me."
She bites her lip, not sure if she was supposed to say that. But she did.
"I mean ... people who can't afford to pay them."

"Well I'd like to hope so." he says after a moment. "I'm a junior doctor, so I have my limits. And I've made mistakes with people in the past."

"No and yes. I have seen people die, but I haven't caused anyone to die. I have come close, but my mistake was spotted."
